Take Charge of Your Health!

Self Management Education

59% of Wisconsin residents have at least one ongoing health condition like arthritis, diabetes, asthma, high blood pressure, depression, heart disease, and others

Self-management programs allow you to learn to thrive, not just survive with your ongoing health conditions!

Attending a self-management workshop will help you find practical ways of dealing with pain and fatigue, discover better nutrition and exercise, understand new treatment choices and learn better ways to talk with your doctors and family about your health. The Healthy Living with Diabetes program is a seven week interactive workshop (six weeks of programming and one week devoted to Q&A). Each workshop meets 2½ hours one time a week and is run by two trained facilitators who have personal experience with ongoing health conditions.
