Wisconsin Family Caregiver ProgramsWisconsin’s Family Caregiver Support Programs provide information, support and resources to people who need help caring for a family member, friend or neighbor.
Caregiver TeleconnectionThis program connects caregivers to support via telephone. The calls are free to anyone. This is a program of the WellMed Charitable Foundation.
AARP Family Caregiver’s Video Guide to Managing Medication46 percent of family caregivers perform medical/nursing tasks such as changing catheters, giving injections, preparing special diets, managing complex medication routines and wound care. These videos are aimed at preparing family caregivers to perform these tasks.

Kenosha County Long-Term Care Workforce Alliance
Kenosha County Long-Term Care Workforce Alliance (Alliance) promotes the well-being of direct care workers. These dedicated individuals provide about 80% of the hands-on care for frail elderly and people with disabilities. Without these committed caregivers, the demand for long-term care services could not be met.

Family CareThe Family Care program offers a wide range of services that meet the needs of consumers wherever they live. A case management program in which a person works with a care manager and a registered nurse. Individuals participate in determining the services they receive including long-term care services, health care services, primary health care coordination, and employment services.
Family Care PartnershipThe Family Care Partnership Program is a case management program that combines health care with long-term support services to increase the ability of people to participate in their personal health care decisions. People receive services in their place of residence through team to based care management including a physician, social worker, registered nurse, and nurse practitioner.

IRIS
Include, Respect, I Self-direct (IRIS) allows for self-directed services and puts the individuals in charge of their own long-term care. Individuals create a plan outlining long-term care supports and services within an assigned monthly budget. Individuals can hire their own service workers directly or may utilize an agency or community provider. Individuals work with an IRIS Consulting Agency (ICA) and a Fiscal Employer Agent (FEA) to coordinate their service delivery.

PACE
Program of All-Inclusive Care for the Elderly (PACE) is a case management program that provides a full range of long-term care, health and medical services, and prescription drugs. This includes the management of a person's Medicaid and Medicare services. People enrolled in PACE receive support determining the services they want from a comprehensive network to help them live in their own home, if possible.
