Children's Community Options Program

Overview

The Children’s Community Options (CCOP) program helps children who are living at home who have substantial limitations in multiple daily activities as a result of one or more of the following disabilities:

  • Developmental disabilities (DD)
  • Severe emotional disturbances (SED)
  • Physical disabilities (PD)

The CCOP Program funding can be used to support a range of different services that assist the family in caring for their child with long-term care needs successfully in the family home. An individualized assessment is conducted with the family of an eligible child to determine what services and/or goods are most appropriate and needed to assist the family care for their child. Funding may be limited and available as a last resort, but you are encouraged to apply.

Some examples of services that could be covered with CCOP funding include:

  • Home modifications
  • Respite care
  • Specialized nutrition and clothing
  • Vehicle modifications
  • Home training / parent courses
  • Other goods and services that are specialized but not otherwise covered by other funding sources.

How to Apply

Parents may contact the eligibility specialist to start the application process by calling 262-605-6599. The eligibility specialist will explain the program benefits and eligibility process, plus assist the family in deciding whether or not the children’s long-term support service is likely to be appropriate for their child. If the parent wishes to apply, the consultant will send the family an application packet.

Once all the application materials are complete, including the receipt of all  required documentation, the eligibility specialist will determine if your child is eligible for the program. If eligible, the family will be connected to a support and service coordinator to begin planning.
