Going Home
From the day you are welcomed aboard, your care is constantly monitored and evaluated to provide for a smooth transition from your rehab program to your future residence. We will help you and your family coordinate your departure within a time frame that meets your unique needs.
Preparations for discharge may include a visit to your home or another setting to ensure that you and your family have the skills and support needed to successfully function after you leave us. You may receive a home program and/or specific instructions or recommendations, which will assist you in the transition.
Our support for your care is only a phone call away. Don’t hesitate to call us if you have any questions that require additional information after you’ve left. We welcome your suggestions as a way of assessing and continually improving the care we provide to you and your family.
