Providing unpaid care for an adult who is ill, frail or disabled?

The Kenosha County Caregiver Coalition and the Aging & Disability Resource Center (ADRC) are interested in learning about the current needs of unpaid caregivers who are caring for an adult who is ill, frail or disabled. If you run errands, set up medications, arrange for services, yard work, handle finances, or clean house for an adult, we’d like to hear from you!

The Survey is confidential and by completing the survey you may also enter your name into a drawing for a $50 Twisted Cuisine Gift Card. Your voice and experience matters!
